Shortage of Primary Care Threatens Health Care System Increasing health care bills, long emergency-room waits and the inability to find a primary care physician just scratch the surface of the problems that patients face daily. Primary care should be the support of any health care system. Countries with appropriate primary care resources score highly when it comes to health outcomes and cost. The U.S. takes the opposite approach by emphasizing the specialist rather than the primary care physician. A recent study analyzed the providers who treat Medicare beneficiaries(老年医保受惠人). The surprising finding was that the average Medicate patient saw a total of seven doctors - two primary care physicians and five specialists - in a given year. Contrary to popular belief, the more physicians taking care of you doesn't guarantee better care. Actually, increasing fragmentation of care results in a corresponding rise in cost and medical errors. How did we take little care of primary care? The key is how doctors are paid. Most physicians are paid whenever they perform a medical service. The more a physician does, regardless of quality or outcome, the better he's reimbursed(返还费用). Moreover, the amount a physician receives leans heavily toward medical or surgical procedures. A specialist who performs a procedure in a 30-minute visit can be paid three times more than a primary care physician using that same 30 minutes to discuss a patient's disease. Combine this fact with annual government threats to randomly cut reimbursements, physicians are faced with no choice but to increase quantity to boost income. Primary care physicians who refuse to compromise quality are either driven out of business or to cash-only practices, further contributing to the decline of primary care. Medical students aren't blind to this action. They know how heavily the reimbursement is against primary care. The recent numbers show that since 1997, newly graduated U.S. medical students who choose primary care as a career have declined by 50%. This trend results in emergency rooms being overwhelmed with patients without regular doctors. How do we fix this problem? It starts with reforming the physician reimbursement system. Remove the pressure for primary care physicians to squeeze in more patients per hour, and reward them for optimally(最佳地) managing their diseases and practicing evidence-based medicine. Make primary care more attractive to medical students by forgiving student loans for those who choose primary care as a career and reconciling the marked difference between specialist and primary care physician salaries. We're at a point where primary care is needed more than ever. Within a few years, the first wave of the 76 million Baby Boomers will become eligible for Medicare. Patients older than 85, who need chronic care most, will rise by 50% this decade. "YOUNG people ought not to be idle (闲置的). It is very bad for them," said Margaret Thatcher in 1984. She was right: there are few worse things that society can do to its young than to leave them ignored. Yet more young people are idle than ever. The International Labour Organization reports that 75 m (m=million) young people globally are looking for a job. World Bank surveys suggest that 262 m young people are economically inactive. The number of young people without a job is nearly as large as the population of America (311 m). Two factors play a big part. First, the long slowdown in the West has reduced demand for labour, and it is easier to put off hiring young people than it is to fire older workers. Second, in emerging economies population growth is the fastest in countries with disordered labour markets, such as India and Egypt. One possible way to settle this problem is to stimulate growth. That is easier said than done in a world suffering from debt, and is anyway a possible answer. The countries where the problem is worst (such as Spain and Egypt) suffered from high youth unemployment even when their economies were growing. Throughout the recession(经济不景气),companies have continued to complain that they cannot find young people with the right skills. This underlines the importance of two other solutions: reforming labour markets and improving education. Youth unemployment is often at its worst in countries with inflexible labour markets. High taxes on hiring, strict rules about firing, high minimum wages: all these help force young people to the street corner. South Africa has some of the highest unemployment, in part because it has powerful trade unions and inflexible rules about hiring and firing. Many countries with high youth unemployment rate have high minimum wages and heavy taxes on labour. India has around 200 laws on work and pay. Across the OECD (经合组织), people who left school at the earliest opportunity are twice as likely to be unemployed as university graduates. But it is unwise to conclude that governments should simply continue increasing the number of people who graduate from university. In both Britain and the United States many people with arts degrees are finding it impossible to get appropriate jobs. In North Africa university graduates are twice as likely to be unemployed as non-graduates. What matters is not just number of years of education people get, but its content. This means expanding the study of science and technology and closing the gap between the world of education and the world of work-for example by upgrading vocational education and by building closer relations between companies and schools. Germany, which has the second lowest level of youth unemployment in the rich world, owns long-history system of vocational schooling and apprenticeships (学徒制). The problem of youth unemployment has been getting worse for several years. But there are at last some reasons for hope. Governments are trying to address the mismatch between education and the labour market. Companies are beginning to take more responsibility for the young. The world has a real chance of introducing an education-and-training revolution worthy of the solution to the problem.(1)In paragraph 2, the author highlighted the high youth unemployment rate by ____ .A. citing quotations and showing figuresB. showing figures and making a comparisonC. giving examples and showing figuresD. making a comparison and giving examples(2)According to the passage, which of the following statements may the author approve of? ____ A. Firms fire more older workers than the young in the economic slowdown.B. As long as the economy grows youth unemployment will soon be settled.C. Powerful trade unions and high minimum wages lead to high employment.D. A flexible labour market is not enough to raise youth employment.(3)In paragraph 6, the author mentioned the German government's effort in order to show ____ .A. deregulating labour markets is the key to settling youth unemploymentB. a flexible labour market can not guarantee low employmentC. governments need to get involved in lowering youth unemploymentD. high minimum wages account for high youth unemployment(4)The passage mainly talks about ____ .A. factors contributing to low youth unemploymentB. possible ways to settle global youth unemploymentC. the need for labour market revolutionD. the relations between education and employment

The increase in international business and in foreign investment has created a need for executives with knowledge of foreign languages and skills in cross-cultural communication. Americans, however, have not been well trained in either area and, consequently, have not enjoyed the same level of success in negotiation in an international arena as have their foreign counterparts. Negotiating is the process of communicating back and forth for the purpose of reaching an agreement. It involves persuasion and compromise, but in order to participate in either one, the negotiators must understand the ways in which people are persuaded and how compromise is reached within the culture of the negotiation. In many international business negotiations abroad, Americans are perceived as wealthy and impersonal. It often appears to the foreign negotiator that the American represents a large multi-million-dollar corporation that can afford to pay the price without bargaining further. The American negotiator’s role becomes that of an impersonal supplier of information and cash. In studies of American negotiators abroad, several traits have been identified that may serve to confirm this stereotypical perception, while undermining the negotiator’s position. Two traits in particular that cause cross-cultural misunderstanding are directness and impatience on the part of the American negotiator. Furthermore, American negotiators often insist on realizing short-term goals. Foreign negotiators, on the other hand, may value the relationship established between negotiators and may be willing to invest time in it for long-term benefits. In order to solidify the relationship, they may opt for indirect interactions without regard for the time involved in getting to know the other negotiator. Clearly, perceptions and differences in values affect the outcomes of negotiations and the success of negotiators. For Americans to play a more effective role in international business negotiations, they must put forth more effort to improve cross-cultural understanding. [共5题](1)What kind of manager is needed in present international business and foreign investment? [本题2分]A. The man who represents a large multi-million-dollar corporation. B. The Harry Potter series, written by J.K. Rowling, is perhaps the most popular set of novels of the modern era. With seven books and many successful films to its name, the series has gathered about 15 billion dollars in sales. How did the series become so popular? The reason can be broken down into several areas. The first book in the series was rejected 12 times before it was picked up by Bloomsbury—a small publisher in England. So receiving this contract was Rowling's first step to success. However, getting a book contract does not ensure the success of a book. The story was soon loved by children and adults alike. In light of this, Bloomsbury Publishing published a second version of the books with “adult” (less colorful and more boring) book covers. This made it easier for a full range of ages to enjoy the series.Another factor that worked like a charm was that the publisher and Rowling herself, through the books, conducted midnight releases, promotions, and pre-ordering more readers. Customers who feared that their local bookstore would run out of copies responded by pre-ordering over 700,000 copies before the July 8, 2000 release. What does the underlined word “releases” (Para. 3) mean?A. The activity that frees or expresses energy or emotion.B. The announcement about the book’s publishing information.C. The sales of books that is available only at midnight.

question or statement.Hot pot is a traditional dish in China. It is believed to date back more than 1,000 years to the time of the Jin Dynasty. Hot pot's origins can be found in the dining practices of Mongolian horsemen who rode across the steppe and into northern China. Legend has it that the Mongols used their helmets as pots to simmer (炖) broth (汤底) over open fires, and cooked chunks of meat in the broth.Hot pot cooking seems to have spread to northern China during the Tang Dynasty. In the regional variations developed with different ingredients such as seafood. In the centuries that witnessed the growth of the Song Dynasty, hot pot moved — and changed — further south, with each successive region adapting it to their local ingredients and tastes. The ingredients in a hot pot vary a lot from region to region. Perhaps the most famous hot pot is the Chongqing or Sichuan hot pot. It features a dark red broth chock-full (塞满了的) of spices, chili peppers, and the uniquely mouth-numbing Sichuan peppers. In Beijing and elsewhere in the north, hot pot broth tends to be mild and, compared to its racy southern "cousins", a little light. In the northeast of China, a kind of local sauerkraut (酸菜) is used to add some tang (强烈的味道), making the broth a bit sour. People can choose a version of hot pot according to their taste.1 Hot pot is believed to date back more than 1,000 years to the time of _______.A. the Jin Dynasty B. the Tang DynastyC. the Song Dynasty D. the Yuan Dynasty
